By general form I assume you mean some variation of
y = ax^2+bx+c
(x+10)^2+(y+5)=16
y+5 = 16 - (x+10)^2
y = -(x+10)^2 - 11
y = -(x^2+20x+100)-11
y = -x^2 - 20x - 111
But I suspect a typo, since there's no reason for both (y+5) and 16
