Active voice is when the subject is doing the action of the verb.

Passive voice is when the subject is NOT doing the action of the verb.

Right.

If a passive sentence tells you who did the action of the verb, it'll be in a "by..." phrase.

In this sentence, the subject is "report" -- which cannot mail anything, right? =)
