Incorrect sentence: The report considers important factors in choosing a medical facility, ratings of services at the urgent care center, reasons why HMO members do not use the center, and offers suggestions for increase in member usage.

I know this has parallelism problems. I have tried rewriting every way I can think of to correct it with no finished product.

Any help would be much appreciated.

Think about the sentence in list form ~

The report considers
- important factors in choosing a medical facility,
- ratings of services at the urgent care center,
- reasons why HMO members do not use the center, and
- offers suggestions for increase in member usage.

<b>After "considers" each element of the series starts with what kind of word -- noun, verb, adjective, or what?

important is an adjective; ratings is a noun; reasons is a noun;offers is a verb.
