Any abbreviations are acceptable as long as both the user and receiver correctly understand their meaning. They can save time and typing.

It seems to me that abbreviations need to be standardized since you never know WHO will ever read those pages. For example, all of my son's medical records (from birth to age 17) had to be sent to the Department of Defense when he was applying for an appt to a military academy. Those records included notes from doctors, nurses, hospitals, phramacies, etc. How would you make sure someone in 1985 could read what was written in 1969 or 1972 or whenever?
