Thank you for using the Jiskha Homework Help Forum. When you need a definition, don't forget a good dictionary! The meaning would depend upon the context, or exactly how the phrase/word is used. If, for example, your teacher said "sign out" for this book = there must be a paper where you write your name so the teacher will know who has the book. As for "take" it's probably "take the book" after you sign out!
