When you say, " What does he look like?" you are asking for a comparison.
You could say, "He looks like an oak tree." Oak trees are thick and tall, so that would be a comparison.
A better way to ask for a description would be to say, "Please describe him."
Then you can say , "He is heavy and tall."
You would not say, "He is a thin man." He is obviously a man; so saying "He is thin." would be enough.
What does he look like?
He is heavy and tall.
He looks like a heavy and tall man.
What is he like?
He is short and overweight.
He is like a thin man.
