First of all, you need to decide if you are using the formal (usted) or familiar (tú) form.
It would help to also know what level of Spanish you have = first, second, third year, etc. Also, if I knew the title, author, year, of the textbook you have, I would know better what sort of expression you would need.
What are you like? (familiar) ¿Cómo eres tú? or simply ¿Cómo eres?
(formal)
¿Cómo es usted? or simply ¿Cómo es?
Another way might be: How do you see yourself? (familiar) ¿Cómo te ves?
(formal)
¿Cómo se ve?
