Let me expand a bit. Parts of the body use the definite article when it is obvious whose leg it is. Here is what we see:

"Doctor Pérez, me duele mucho la pierna. ¡Ay!"
translation: Dr. Pérez, MY leg hurts a lot. OW!

"Miguel, ¿te duele _____ ?"
translation: Michael, does........hurt you?

a. the (your) right or left? The gender is feminine singular, just like "pierna/leg"
b. the (your) right or left? BUT here the gender is masculine, singular.
c. the purple or the pink (I sincerely doubt this is referring to anyone's leg.)
d. the (your) fifth or seventh = unclear as to the antecedent

Now, this is taken out of context. If we were privy to the entire conversation, it might be clear that the doctor is referring to PART of the leg, but only seeing the item alone, the specified noun should be LEG.
