Classical Conditioning.
1)Terell feels ill every time he sees a Wendy's restaurant sign. Two weeks ago while on his lunch break, he went to Wendy's and ate a chicken sandwich. He was not aware that the chicken was spoiled.
a. Name the Unconditioned Stimulus(US): Spoiled chicken sandwich
b. Name the conditioned stimulus(CS): Every time he sees a Wendy's sign.
c. Name the Unconditioned Response(UR): Terell feeling ill
d. Name the conditioned response (CR): Terell feeling ill.
I put in what I thought the answers may be but I am not sure if I am doing this right. Any help would be appreciated! Thank you

Looks good to me, but double check your text materials. I question how his response can be both conditioned and unconditioned, but I have not read your text.
A UR is in response to a US, while a CR is in response to a CS. They can both be the same, but may differ in magnitude.
