I have a couple questions I need help on.
Which model is most appropriate for the data set?

on the first sets, each increase in x results in y being increased by 5 (assuming you have a typo in the third set, and it should read (1,15). So linear.
divide by 2
1+4x+4x^2=0
(1+2x)^2=0
x= -1/2, -1/2 so one real solution.
divide by 2

(1+2x)^2=0
x= -1/2, -1/2 so one real solution.
