How do you know if a quadratic is a perfect square?

When written in ax^2 + bx + c form, it is a perfect square IF b^2 = 4 a c.
In that case, it is the square of
(a)^1/2 * (x + b/(2a)]
In that case, it is the square of
(a)^1/2 * (x + b/(2a)]
