A 16 ounce container of a solvent must be mixed with 48 ounces of water. What fraction of solvent is there in the total mixture (lowest terms)?

16/48 = 2/6= 1/3

The TOTAL mixture is 16 oz. solvent plus 48 oz. water (16+48) so your denominator is not correct. Put the 16 over this total then reduce and you will have your answer.
