In the lab or industrially.
NaCl can be produced in the lab with HCl and NaOH. Industrially, it is mined, either from the salt domes (Louisiana has bunches) or from the Salt Lake City, Utah area.

Hydrogen in the lab is produced with an active metal and water or and acid (depends upon which metal is used).
Industrially, I think most of it is done through steam reforming of natural gas or by electrolysis (a more expensive process).
