Thank you for using the Jiskha Homework Help Forum. Since you don't say which tense, here is the Present Indicative:

decidir = to decide
yo decido = I decide, I do decide, I am deciding
tú decides
él/ella/usted decide
nosotros/nosotras decidimos
ellos/ellas/ustedes deciden

Most textbooks don't include vosotros/vosotras so let me know if you need that form.

Note that there are 3 possible English translations for each form, except, of course, the infinitive form which has no subject or time.

That word is an already-conjugated form of the infinitive verb "decidir". It means "they decide" or, for Ustedes (formal plural you) "you decide".

The Mood means either the Present Indicative (which I gave you) or Conditional, or Subjunctive. What do YOU mean by "mood?" As for voice, it is active or passive. What do YOU mean by "voice?" The conjugation is what I gave you above with the Present Indicative.
