How do I fix this sentence?
My patient replied back to me, “I am good now and Thank You very much.”

Nikki is right. But -- you need a comma after "now."

replied back = repetitive; "re" means "back"
Better to make the word in quotations into two sentences. It reads more smoothly. Also, watch capitalization.
