Classical Conditioning.
1)Terell feels ill every time he sees a Wendy's restaurant sign. Two weeks ago while on his lunch break, he went to Wendy's and ate a chicken sandwich. He was not aware that the chicken was spoiled.

Looks good to me, but double check your text materials. I question how his response can be both conditioned and unconditioned, but I have not read your text.

A UR is in response to a US, while a CR is in response to a CS. They can both be the same, but may differ in magnitude.
