the client has Parkinson disease. he tries to administer his own oral medication . his wife usually help him, is often not home for his afternoon does because your visit her respite time

I will help the client as long as the medication needs to be administered on the right time, right route and dosage that has been specified in the record sheets.

A - The client find it difficult to administer his afternoon dose, due to his wife is often not home.
C - I will contact my supervisor immediately and inform him/her that the client is having difficulty taking his afternoon dose(medication).
T - I will talk to his wife if present.
I - I will help the client as long as the medication needs to be administered on right time, right route and dosage that has been specified in the record sheets.
O - I will open the chart and document what I have done.
N - I will come up with a note to follow-up next visit to see if client is still having problems.
