3. I have never used a cell phone in class.

4. I have played ball in my classroom before.

5. I have ever played ball in my classroom.

(Are they all grammatical? Is #5 a corect expression?)

#5. no. I have never...is fine and you might hear the question: "Have you ever..." but not as it is. You could say: "I have always..."
