I would think it'd be the patient. I can't tell you how many times other people have misspelled my name. (Only I seem to be able to do that correctly!) And if they can't get my name straight, how can anyone be sure other data is correct either?
The answer is D. The person who creates the data in the record. It wouldn't be the patient
