What would cause an image to appear blurred?

Photo blurred? Out of focus, or the camera moved, or the subject moved, or the camera was set at to low a shutter speed.
no, i mean on/ in a microscope.
It is out of focus.
Dude, really? "It's out of focus".... well duh!
Just kidding, it probably is the lense. You could've scratched it in some way.
