About 1/3rd of the way down the page, put the title of the book report -- not the book's title, but the title of the student's report (which can include the book's title but not BE the title).
two spaces down = the word "by"
two more spaces down = student's name
2/3rds of the way down from the top are three things, either centered or off to the right:
Due date
Name of class
Name of teacher
