Every complete thought must have a subject and a verb.
The verbs are "was ... incised" and "was lengthened.".
"was incised" is the action that is performed on the "external oblique fascia," and "was lengthened" is the action performed on the incision.
"using" is not a verb because it describes how.
The external oblique fascia was then incised, and the incision was lengthened in the direction of the fibers using Metzenbaum scissors.
