(Are both the same?) No. Sentence 2 is not standard phrasing. You could have 2 read like this: "I don't completely agree with you."

3-1. I don't agree with you at all.
(Are both the same?) Yes, but put "entirely" right after "I" (next to the verb it's modifying).

4. I don't agree with you entirely.
4-1. I agree part of what you said.
(Are both the same?) Yes, although 4 would read better if you put "entirely" right after "don't".
