#s 1, 2, and 3 mean approximately the same thing when referring to health, but #1 could have a broader meaning (water, juice, tea ... ). I think #3 is the best one to use in a unit on living a healthy life. Or you could say, "Don't drink alcohol to excess."

#4 can mean any of the other three, and yes, depending on the context. =)
