Assuming a typo, and that you mean

y = 1/2 x + 5

the slope is 1/2. Now, using the point-slope form, we want the line

y+3 = 1/2 (x-4)
or
y = 1/2 x -5

A perpendicular line will have slope -1/(1/2) = -2.

use that as you will.
