I am so confused on this assignment in HSM/220(Budget Matrix-Appendix B) and wondering if I am even close to being correct on showing where to place the X's for a Functional Budget,Line-item Budget & Total Program Budget) before I submit this assignment.

and I cannot copy it and send it just the way it shows it on the assignment grid. The first category is: Functional budget to determine personnel costs per member enrolled, second category is: line-item budget to determine non-salary costs and third category is Total Program Budget.
I am sincerely not asking for the answers I am just asking for some guidance as to: if I am even close to completing the assignment correctly ?

Thanks for your help, as I am really confused and I have read the reading that goes with this assignment so many times that I have lost count and stressing out, badly here.

Without being able to see the real chart, it's very hard to tell. You may get some ideas from the search results in the link above.

You are right, though, that you need to set up
... all personnel costs first (salaries/wages, FICA, any other taxes and/or fees)
... then the costs for the building(s), including rent/lease, utilities, etc.
... and finally (last in priority!), machines, supplies, and everything else.
