1)I know that the sentence "Why are you always late" correct, but is it possible to say "Why are you always being late" to show that it happens very often?
2)Do you think it's possible to use "every time" in the same context "Why are you every time late?" or "Why are you late every time?"
3)Is it OK to say "He is not late ever" meaning "he is never late"?

The word "being" is not needed in that sentence; "always" takes care of the emphasis.

2)Do you think it's possible to use "every time" in the same context "Why are you every time late?" or "Why are you late every time?"
The first sentence is very awkward, but the second sentence has the adverbial phrase "every time" in the correct place.

3)Is it OK to say "He is not late ever" meaning "he is never late"?
No ... "He is never late" is all you need to say.
