This is just to get an idea, so when you get to High School, we know what kind of support we need to give you, as counselors, and planning your High School schedule.
Once you fill out this form, tell me that in this box below.
This will be a separate grade in the gradebook, and will be graded separately, so please make sure you complete this task.
