According to MLA Style guidelines, information should be listed on a works cited page for a book by one author as follows:

Author's Last Name, First Name. Book Title. Publisher, Year of Publication.

For example:

Smith, John. The Power of Habit. Random House, 2012.

Note: The first line of each citation should begin at the left margin, and the second and subsequent lines should be indented 0.5 inches (known as a hanging indent).
