The poet's last name.
if so, would the title be in italics then comma then the poet? or JUST the poet's last name?
No, the title is included in the Works Cited and would not be included in the parentheses.

Best of all, is to name the poem (and put the title in quotation marks) at the beginning of the sentence which introduces the quotation from the poem itself.
