Use brackets to eliminate any ambiguity.
Your common denominator is 2x-10, so
[(-7x+3)(2x-10) + (4x^2 + 1)]/(2x-10)
= [-14x^2+70x+6x-30+4x^2+1]/(2x-10)
= (-10x^2 +76x - 29)/(2x-10)

-7x+3+4^2+1/2 x-10
