4) Dr. Jack, with the assistance of Dr. Jill and Dr. Pail, IS performing the surgery.
5) Past medical history shws the patient's mother currently HAD breast cancer.

Everything looks good except #3, it is supposed to be Was.

Liz is wrong.

The subject, "costs," is plural so the plural verb, "were" is correct.

Lori -- number 5 is incorrect.
