why would it be difficult for a small community hospital or a practicing physician to survive in today's health care climate?

Costs of everything in the medical field -- salaries, benefits, insurance, equipment, pharmaceuticals, etc. -- are so high that doctors and hospitals need the economy of large scale purchasing to stay afloat.
