1.) Should there be a comma before "is" in the following sentence below?
She gave birth to her first child by her uncle at age 14 in 1990 is showing a sign of being raped.

2.) The psychologist help Alesha to break through her issues by using cognitive-behavioral therapy.
Is "to" neccessary or should take it out?

Sentence 1 is not well done. You need to put a period after "1990" and then supply a subject for the verb "is showing."

Sentence 2 is fine, and yes, you can omit "to" if you wish.

If she was 14 when she became pregnant by her uncle, she was raped. She was below the age of consent and therefore, legally was raped.

You might say,

She was raped by her uncle and gave birth to his baby when she was 14.
