The best choice is option 2. In MLA the top-left (on the first page only) should list, on separate lines:
- Your name
- Instructor’s name
- Course name (or class/period)
- Date (format: day month year — e.g., 17 December 2025, no comma)

Also note: a running header with your last name and page number goes in the top-right of every page.
