Everything looks good except you should delete "web." It's not needed. With the URL there, it's obviously from the Internet.

According to what style? According to MLA, you may only put down web along with the rest. You no longer have to put down site addresses unless you feel like it or there is some good reason for doing so. I have always wished that we could all post hyperlinks here, but oh well. . .
