I always worry the night before a big test. Then I can't sleep well.

(Wha is the meaning of 'then' here? Is 'then' 'at that time, or 'after that'?)

I agree that the interpretation could be either. However, in this case it only makes sense it you consider "then" to mean "at that time", on the night before the test.
