It depends on the state laws. It is hard to believe in any state, if it went to court, the employer is NOT responsible to pay, either through state run disability claims and work injury programs, or through self-insurance, or through the employeers own pocket. The employee is NOT responsible, so that eliminates answers a, b, c.
