All are grammatically correct, but what Sra is telling you is that the question in #2 is more likely to get a physical description or a character description, such as these:
She has a hot temper.
She is easy-going.
She is a very helpful person.
She is tall and thin like a beanpole.
etc.

2. What is she like?
She looks happy.
3. How does she look?
She looks surprised.
4. What does her appearance look like?
She looks upset.
(Are the questions and answers grammatical?)

#2 = What is she like? This more apt to get a physical description.
