is this correct statement

After undergoing a chemical reaction substances no longer exist in their original form? If not explain.

I think it is correct but then i think this might be the definition for a chemical change ( not reaction )

The staement is correct. A new chemical compound is formed in a chemical reaction, and all chemical and physical properties change
