The sqrt of 16 is 4. The sqrt of 4 is 2.And just keep the sqrt of (2) the same.(:

the way you typed it ...
sqrt (16)/sqrt (4) + sqrt (2)
= 4/2 + √2
= 2 + √2 which would be none of the anwers

If you meant
sqrt (16)/(sqrt (4) + sqrt (2))
= 4/(2 + √2)
= 4/(2 + √2) * (2-√2)/(2-√2)
= (8-4√2)/(4-2)
= 4 - 2√2
