If we write it this way, so that the atomic number is first and the mass number is second, it will be a little less confusing. In general, however, just keep the numbers so the subscripts add up on each side and the superscripts add up on each side.
13Al25 + 15P30 = 15P30 + on1 + 13Al24
How do you know it's P. Look on the periodic table for atomic number 15 and that's it.
