2. Tom is spoken well of by him.
3. Tom is well spoken of by him.
(Which one is the active voice of #1?)

4. She took care of the baby.
5. The baby was taken care of by her.
6. Care was taken of the baby by her.

(#5 is the passive of #4. What about #6? Is # 6 also the passive voice of #4 and grammatical?)

#1 is in active voice; its passive can be either 2 or 3, but 2 reads more smoothly.

Yes, 5 is the passive of 4. #6 is worded very awkwardly and is not used.
