First, find the slope.
slope= (y2-y1)/(x2-x1)

Second, chose one of your coordinates and plug in...
[y - y1(or y2)]= m[x - x1 (or x2)]

standard form:
For example, 3x + 4y =10 (notice how the x and y are on one side)
