First you need to determine the topic of your questionnaire and what you want to find out. researching the variables related to your topic will help you to determine what questions to ask.
For validity, compare results to some other criterion of the variable that concerns you. Depending on what type of reliability to want, you can do repeat measures with the same subjects.
