What are some good questions to be asked during an interview with a counsellor?

Wouldn't it depend on who was asking the questions (client or counsellor). Wouldn't it depend on the type counsllor (legal, psychological, career, medical, weight loss)

The counselor could ask:

"Why have you come to see me?" (Unless this was already answered in making the appointment)

Depending on the answer to this question, further questioning would be varied. "When did this (problem, these problems) first start (become evident)?" "How have you dealt with this problem previously?" "What happened when you tried to deal with the problem?"

As bobpursley indicated, further questions depend on the type of counselor and the problem being presented.
