4 cm = 0.04 meter
y = 0.04 sin (2 pi t/T)
when 200 pi t = 2 pi , t =T
T =0.01 second
say x = A sin (2pi t/T)
then
v = 2 pi/T A cos(2 pi t/T)
a = -(2 pi/T)^2 A sin (2pi t/T) = -(2 pi/T) x
I think you can plug in x = 0, x = A = 0.04
y = 0.04 sin (2 pi t/T)
