I don't think A is correct because the question mark should be a comma.
Example:
"How was the game," asked Remy.

So C?
Answered by Ami
Your correct answer is B.
A question mark should lead with a capitalized letter, so it's not A.
C is definitely not correct, because of the same issue as A.
Quotation marks are in the wrong place and incorrect capitalization on D.
