If our actual yield from this reaction was 12 grams of sodium chloride (NaCl),
what would our percent yield for this reaction be?

Not enough information. What was the reaction? How much of the reagents did you start with. OR, what was the theoretical yield. If you know the theoretical yield (TY) and you have the actual yield (AY) then
% yield = 100*(AY/TY)
% yield = 100*(AY/TY)
