A baby is in a cradle. One of the boys is looking at the baby. He said, "_________"

1. It is smiling.
2. He is smiling.
3. She is smiling.

Wich expression can he use? If the boy doesn't know whether the baby is a girl or a boy, does he have to say as in #1?

Once a baby is born, it's best not to use "it"! I'd use "he" and then wait to see if the parents correct me.
